Consent to the processing of personal data and instruction to the Data Provider

By completing and sending this form, I grant INTEROMODAL NEWS Sp. z o.o. 50-576 Wrocław, ul. Czerniawska 2A/27 (hereinafter “Administrator”) consent to the processing of my personal data, provided the following conditions:

Personal data:

name and surname, titles,
company name
functions
e-mail address
phone number

The purpose of personal data processing is:

administration relating to the organisation of professional and social events

sending invitations and other messages related to the Administrator’s activities, including those cooperating with the Administrator (general sponsors, partners, co-organisers)

Personal data processing time:

The data will be processed for a period of 10 years.

By completing and sending the form to the Administrator, the Data Provider agrees to the processing of personal data and declares that he has been duly instructed by the Administrator on the processing and protection of personal data *, and that the above personal data is accurate and true, and provided to the Administrator voluntarily.

Information for the Data Provider

According to Art. 13 of Regulation (EU) No. 2016/679 of the European Parliament and of the Council of 27 April 2016. The General Data Protection Regulation (hereinafter referred to as the “Regulation”) informs that:

The Data Provider’s personal data will be processed on the basis of voluntary consent, under the following conditions:

The reason for providing personal data by the Data Provider is the Data Provider’s interest in sending information and invitations or business offers to organised events related to the Administrator’s activity, which would not be possible without providing such data
The administrator has not appointed a data protection officer, has not authorised any data processor to process personal data, and has not appointed a representative to perform the obligations arising from the regulation,
The Administrator does not intend to transfer the personal data of the Data Provider to third countries, international organisations or other organisations than the above-mentioned third parties
The data provider has the right to revoke his consent to the processing of personal data at any time, the right to request the Administrator to access his personal data in order to correct or delete personal data or limit processing, opposes processing, has the right to transfer this data to another Administrator as well as the right to submit a complaint to the Office for Personal Data Protection, if it considers that the Administrator acts in violation of the Regulations when processing personal data.

At the events organised as part of the INTERMODAL IN POLAND series, photos and video recordings will be taken, which the Organiser will then make available to the participants of the event, and will also be used for marketing purposes.

By submitting your application and participation, you agree to the publication of photos and videos from this event.
